This forms your contract with Pet Poppins Dog Training.
1. On booking payment is upfront and due 48hrs after invoice sent via email. Payments are made via bank transfer. Payment plans can be discussed in advance of booking.
2. At least 48hrs notice must be given when rearranging an appointment.
3. Any appointments cancelled with less than 48hrs notice will be lost or will have to be paid for if rearranged. Please tick the box on the form to demonstrate that you understand this.
4. All payments are non refundable once booked and training communication has commenced.
5. All training sessions once paid for need to be completed within six months or they will be lost.
6. The information on your assessment form will be used as a record of your dog’s history and assist in planning future training objectives. It will not be passed on to any third parties.
7.Pet Poppins Dog Training occasionally takes photo’s and video’s to document training progress and with permission may use them for promotional material, social media or the Pet Poppins website to evidence the work undertaken by Pet Poppins Dog Training. 8. Consent to communicate via email must be sought due to GDPR laws. Please tick the box on the form to consent to me sharing training information with you.
